A smile that has been ruined by disease or traumatic injury need not be permanent. There are still many ways to restore that smile thanks to modern dental treatments. One of these treatments, which is a relatively simple procedure that requires neither surgery nor a long recovery period, is Restorative Dentistry, also known as Tooth Bonding.

“Tooth Bonding is a simple dental procedure where colored material is placed on the surface of a tooth, for the purpose of repairing a decayed tooth or a tooth that was chipped due to occlusal force or an accident; or to mask discolored teeth.

“Composite resin material is also used to replace, or as an alternative to, a metal filling. Another use of composite filling is to cover a root part of the tooth which has been exposed due to excessive tooth brushing force and gum recession,” said Dr. Malony L. Santos of Dental World Manila.

Dr. Santos added that restorations done using composite resin material can help a patient recover the function of his or her natural tooth. Furthermore, tooth restorations done through composite materials look more aesthetically pleasing compared to metal fillings.

Tooth Bonding using composite restoration can treat simple to complex cases of tooth damage. An adjunct such as retentive pins can be used to reinforce the strength of a composite filling in cases two walls of the affected tooth are being reinforced or built up.

Tooth restorations using this treatment can last several years before they need to be repaired or replaced due to stains. Adherence of the material depends on the extent of the restoration work and on occlusal force in the affected area. Once the procedure is done, a patient may be asked to return after 24 hours so the dentist can trim down any sharp edges.

While tooth restorations through Tooth Bonding can restore a natural tooth’s function, the composite material is not as strong as a natural tooth. Chewing food ordinary food is all right but if a patient bites on hard candy, an ice chip, or some other tough material, the composite can break and dislodge, in which case it would need replacement. Dr. Santos mentioned a few reminders for patients undergoing Restorative Dentistry/Tooth Binding.

“The patient and the dentist must discuss the limits that the composite restorations can do, so the patient will have realistic expectations from the procedure. And like all dental procedures, Restorative Dentistry begins with oral prophylaxis or tooth cleaning.

“Composite restorations can be stained by certain foods and beverages like tea or chocolates. Cigarette smoking also stains them. To minimize this, a patient should avoid such foods and smoking as much as possible. Regular brushing using a soft-bristled toothbrush, plus regular dental visits every 6 months, for oral prophylaxis and tooth polishing, are essential—not.

More About Dental Implants

Tooth Implants are made to replace the lost natural teeth due to decay, infection, trauma and periodontal disease. It is an artificial replacement for natural teeth. It is designed in such a way to mimic roots of a natural tooth. Dental implants are made of titanium metal, the only kind of metal that is compatible to human body.

Before implants are placed and inserted into the bone of upper or lower jaw, your implant dentist will require a thorough diagnosis to see if you are a candidate to receive the implants to check if you have quality bone enough to hold the implants. Normally, this will include taking of panoramic x-ray, intraoral pictures, impression to make the cast and to some extent a CBCT Scan that is now popular with Dental Implantologist for being accurate and precise when it comes to diagnosis. Unfortunately, this CBCT (Cone Beam Computed Tomography) Dental Scan is not available in any dental clinic because of the high price of this machine.

A. Conventional Dental Implants

The treatment time for conventional type is around 3 to 6 months in 5 to 7 dental appointments with your implant dentist. This type of implant will attach to your bone in the process called Osseointegration. After the implants are totally integrated to your bone that is the only time your implant dentist can do the final prosthesis or the porcelain crown.

Some patients are even required to undergo bone grafting procedure if they do not have enough bone to support the implants. This will complicate the implant procedure and treatment time becomes longer. It can last even for more than a year.
Some patients have very low sinus for being edentulous (without teeth) for so many years. This will also complicate implants placement on the upper jaw and this might require bone grafting and sinus lifting procedures before implants can be placed.

One advantage of tooth implants compared to having removable dentures or porcelain crown or bridge is that it would prevent shrinkage of your bone after teeth have been extracted.

B. Mini dental Implants

This is the smaller type of tooth implants which is only 1.8mm to 3mm compared to 3.3mm to 5mm of conventional type.This is specifically made for patients whose ridge or bone have thinned because of being no teeth for a long period of time. Mini implants becomes an alternative dental treatment for patients who wants to have implants but cannot be accommodated by the conventional type.

One advantage of mini teeth implants is the shorter treatment time and practically no downtime. Mini dental implants can be placed in just one day with virtually no discomfort. It is just being screwed into your bone with no cutting of the gums and stiches to remove.

Some implant dentists will use mini dental implants to hold porcelain crown and bridges but some would discouraged this practice as this might cause  problem later on. Remember that there is no actual integration of implants into your bone unlike with the conventional dental implants thus there is the tendency for mini implants to get loose eventually.

One good use of mini dental implants is the stabilization of upper or lower complete dentures. Normally, four implants can be placed on upper or lower arch. This procedure can be done in 1 to 2 hours in one appointment with your implant dentist and presto you have a stable complete denture that you can use right away without discomfort.

My advice for patients who are thinking of having implants is to not wait too long and consult an implantologist  at once after you have undergone extraction. One to two months after tooth extraction is the ideal time to place an implants. Implants placement is rather an easy procedure so do not wait until your bones are all gone. You can save a lot of money, time and the inconvinience of having several surgical procedures if it can be done earlier.

One to five implants placement are usually done in the dental office set up but for more implants with bone grafting and sinus lifting procedures, my advice is to have it done inside the operating room under general anesthesia.

Even so, Dr. Santos assures, those who are not naturally gifted with a perfect set of teeth can achieve it. With 16 years of dental practice under his belt and a long list of high-profile clients, Dr. Santos was chosen as the head judge for the Best Smile award during the 2008 Miss Earth pageant.

All things considered, Mother Nature has been generous to all the 85 candidates, but Dr. Santos and 3 associates from Dental World Manila carefully evaluated each one to determine which of the ingénues wins the award and the P15,000 gift certificate from his renowned clinic.

The criteria the judges used very well define what a perfect smile is: well-aligned and pearly white teeth, complete dentition (no missing teeth), caries-free, good bite, a smile not reaching the gum line, and pinkish gums. After deliberating, the judges declared Miss Brazil the winner. On coronation night, Miss Brazil sailed through to the finals and won Miss Fire.

A. Teeth Whitening

Teeth Whitening is a common and popular chemical process used to whiten teeth. Some people get their teeth bleached to make stains disappear while other just wants a whiter shade. Discoloration occurs in the enamel and can be caused by medication, coffee, tea and cigarette. Discoloration also can be caused by your genetic make-up or simply getting older. Bleaching can be performed by your dentist in the office or under dental supervision, at home. Many patients enjoy bleaching at home because it is more convenient. Treatment begins when your dentist creates a custom mouthpiece to ensure the correct amount of whitening solution – 10 percent to 20 percent carbamide peroxide-is used and that your teeth are properly exposed. Typically whitening at home takes two or three weeks, depending on the desired shade you wish to achieve. Whitening in the office may call for two to six 45-minute visits to your dentist’s office.

B. Tooth Bonding

Tooth Bonding is tooth-colored material used to fill in gaps or change the color of teeth. Requiring a single office visit, bonding lasts several years. Bonding is more susceptible to staining or chipping than other forms of restoration. When teeth are chipped or slightly decayed, bonded composite resins may be the material of choice. Tooth Bonding also is used as a tooth-colored filling for small cavities and broken or chipped surfaces. Additionally, it can be used to close spaces between teeth or caver the entire outside surface of a tooth to change its color and shape.

C. Dental Crowns

Dental Crowns also known as Dental Caps are used in cases where other procedures will not be effective. Dental Crowns have the longest life expectancy of all dental cosmetics restorations, but are the most time consuming.

D. Dental Veneers

Dental Veneers or laminates are placed over the front teeth to change color or shape of your teeth. Porcelain Veneers are used on teeth with uneven surfaces or are chipped, discolored, oddly shaped, unevenly spaced or crooked. Porcelain Veneers are thin pieces of porcelain or plastic cemented over the front of your teeth. Little or no anesthesia is needed. Porcelain Veneers are used to treat some of the same problems as bonding. This treatment is an alternative to dental crowns, which are most expensive. The dental procedure requires your dentist to take an Impression of your tooth. Before the custom-made dental veneer is glued directly onto the tooth, your dentist will lightly buff the tooth to compensate for the added thickness of the dental veneer. Once the cement is between the dental veneer and your tooth, a light beam is used to harden it. Porcelain veneers require more than one visit because they are fabricated in a laboratory.Dental Veneers have a longer life expectancy and color stability than bonding.

E. Teeth Contouring and Reshaping

Tooth reshaping and contouring, is a procedure to correct crooked teeth, chipped teeth, cracked teeth or even over lapping teeth in a single session. Tooth reshaping and dental contouring, is commonly used to alter the length, shape or position of your teeth. Contouring teeth may also help correct small problems with bite.It are common for bonding to be combined with tooth reshaping.

This procedure is ideal for candidates with normal healthy teeth, but who want subtle changes to their smile. Your dentist will take X-rays to evaluate the size and locations of the pulp of each tooth ensure that there’s enough bone between the teeth to support them.

Dr. Malony L. Santos came from a family of lawyers from Bulacan: his grandfather, his father, and a cousin are all lawyers. Naturally, the interest ran through him as well, citing Social Studies and Current Events as his favorite subjects in high school and Jurisprudence in college. But during summer vacations, he would assist his mother in their fertilizer store as an errand boy. This sparked a passion in Dr. Santos and he has never looked back.

Years later, already a dentist, Dr. Santos chose to set up his practice in Manila not in the province, against what his family wanted. He believed that dentistry is more lucrative and sophisticated in the big city, and his room for growth as a dentist would be limited if he were to stay in Bulacan.

With a few basic dental equipment provided by his parents, Dr. Malony rented a small, existing clinic at the Virra Mall in Greenhills, together with a classmate and business partner. Neophytes as they were, they encountered many difficulties upon opening their clinic. They relied on walk-in customers, but there weren’t a lot of people that pass by their location. Soon enough, sustaining the clinic began to be a problem. His dental partner eventually got discouraged, gave up, and pursued another career.

Determined to see his dental practice come into fruition but still equipped with his basic dental equipment, Dr. Santos gathered what was left of his savings and searched for another possible location for his dental clinic. As luck and great talent would have it, he ended up at the St. Luke’s Medical Center in Quezon City. Armed with the lessons he learned from his previous venture, he slowly rose back up and ultimately became affiliated with the renowned hospital.

As the modest income from his clinic gradually grew, he was able to invest on the latest dental equipment to upgrade his facilities. He complemented this with various dental training courses to expand his expertise, all while running the clinic by himself, aided only by his secretary.

The hard work paid off, and his dental practice steadily flourished. Soon, he was able to expand his manpower resource. With more staff members to assist him in the clinic, Dr. Malony was able to open a branch at the SM Mall of Asia. Initially, He had difficulty maintaining the MOA branch, since it is a mall-based clinic rather than at a hospital. Some privileges and conveniences are unique to St. Luke’s. For example, Dr. Santos had to comply with stricter rules and regulations from the mall management, particularly with regards to clinic hours. In spite of this, he plowed on and soon learned the ropes of effective branch maintenance.

While the mall location had more walk-in customers, he wanted to market his clinic more aggressively. He was soon listed on the Yellow Pages, and Dr. Malony launched his own website promoting his services. These benefited him greatly. Balikbayans had read about his clinic. They came home for his dental services, and now comprise Dr. Malony’s main clientele.

Another opportunity for expansion opened up, this time, in Glorietta in Makati City called the Glorietta Dental Center. Adept and much more experienced now in handling a mall clinic, Dr. Malony re-upgraded his dental facilities and equipment, transforming his practice into a highly profitable business.

With 3 branches, 8 associate dentists, and 6 auxiliary staff members, Dr. Malony achieved his goal of a successful Manila dental clinic. He plans to open 3 more dental centers.

For Dr. Santos, success in the field comes from good dental office management skills and a keen eye for details. He shares, “At all times, do your best, and exhaust every possible means to make your patients happy and satisfied”.

Dr. Malony is a graduate of Centro Escolar University in Manila (1994). He took his dental externship at the Veterans Memorial Medical Center and finished his preceptorship courses at The Asian-American Academy of Functional Orthodontics and TMJ (1996), and the Philippine Academy of Esthetic Dentistry (2004). By 2007, he had finished a 14-month course on Osseointegration Implants under the Philippine Society of Osseointegration.

He had his training on Implants at IMTEC SENDAX and learned of the Mini-dental Implant System. He also had his post-graduate course in Periodontics.
